The evaluation process may include actuarial tools, review of past medical records, and consultation with any applicable outside providers or collateral contacts with your informed consent.
Following confidentiality laws, diagnosis and recommendations can be provided to a specific professional or entity (e.g. surgeon, insurance, courts / legal entities, employer human resources, etc.) upon written consent and request.
